Utah Veterans Massage (“we,” “us,” “our”) provides massage therapy services to veterans, primarily through the VA Community Care Network. This policy explains what information we collect, how we use it, and the choices you have.
Information We Collect
- Contact information you or the VA provide to us: name, phone number, email address, and mailing address.
- Appointment and scheduling information: appointment dates, times, locations, and your massage therapist.
- Health information you share with us in the course of care, which is protected health information (PHI) handled in accordance with HIPAA. Our HIPAA Notice of Privacy Practices, available at our offices, governs how we use and disclose PHI.
How We Use Your Information
We use your information to schedule and provide massage therapy services, communicate with you about your appointments and care, coordinate with the VA and your referring providers, and meet our legal and insurance-billing obligations.
Mobile Information & Text Messaging
If you consent to receive text messages from Utah Veterans Massage, we will send you appointment reminders, scheduling messages, and replies to your questions.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Sharing of Information
We do not sell your personal information. We share information only with: (a) the VA and your referring providers as part of your care; (b) insurance payers and clearinghouses for billing; (c) service providers who support our operations (such as our scheduling and communications systems), who are bound to protect it; and (d) as required by law.
Your Choices
You may opt out of text messages at any time by replying STOP. You may request that we communicate with you by a different method, and we will accommodate reasonable requests.
